Prior to your first appointment…
In order to expedite your first appointment, please download our initial intake form prior to your arrival. It may take up to 20 minutes to complete these forms, and we want you to get the maximum benefits from your time here. If the forms are not completed before your appointment, please arrive at least 20 minutes prior to your scheduled appointment. We need these thoroughly completed in order to do a comprehensive evaluation.
We request that you bring with you any recent laboratory tests, medical reports and bottles or samples of all medications, herbs, homeopathics and nutritional supplements [vitamins, etc.] that you are presently taking. This is extremely helpful for your initial evaluation. We encourage you to prepare any written questions ahead of time and bring them to your initial appointment.
Payment Information: Full payment in the form of cash, check or credit card is
due at the time of service. We are willing to bill your insurance company if
they are willing.. Otherwise you will be responsible for payment at the
time of service and we will provide you will a service summary that you can
submit for reimbursement.
Missed Appointment: A 24-hour cancellation policy is necessary, so that I may
have the opportunity to accommodate other patients. Canceled office visits or
phone consults without a 24-hour notice will result in the visit charge to you.
Phone Calls: As a courtesy, I am happy to answer questions by phone or email at
any time that I am available. However, telephone calls that extend more than five
minutes or extensive emails will be billed at the usual rate.
When you wish to refill your medicinary items, please call in advance
so that your waiting time will be minimized. If your medicinary item is specially ordered
for you please call 1 week in advance.
